Welcome to the MXR family. To ensure a seamless start to our partnership, we require all new clients to complete our standard setup documentation. This includes our credit application and general account information, which allows us to quickly process your orders for parts, service, or new X-ray equipment. Please download the form below and return it to your representative to begin the activation process.

By clicking the download link and installing TeamViewer, you confirm your understanding and acceptance of these terms. By proceeding with the download and installation of TeamViewer, the Customer acknowledges and agrees to the following:
Third-Party Application: TeamViewer is developed, owned, and maintained by TeamViewer GmbH, not by MXR Imaging. MXR Imaging has no control over the software's development, security, and/or functionality.
Voluntary Installation for Support: MXR Imaging may request that the Customer install TeamViewer on a Customer workstation solely for the purpose of providing remote technical support services as mutually agreed upon. Installation of the TeamViewer software is at the Customer's sole discretion and is not mandatory for all support interactions.
No Liability for Software Issues: The Customer understands and accepts that MXR Imaging is not responsible or liable for any direct, indirect, incidental, special, consequential, or punitive damages, including but not limited to, loss of use, loss of data, loss of profits, system malfunctions, or any other issues or vulnerabilities arising from the use, performance, delay in use/performance, or installation of the TeamViewer software itself whether based on contract, tort, negligence, strict liability, or otherwise, even if the Customer has been advised of the possibility of damages. Some states/jurisdictions do not allow the exclusion or limitation of liability for consequential or incidental damage, the above limitation may not apply to those Customers, if the Customer is dissatisfied with any portion of TeamViewer, or with any of the terms and conditions contained therein, the Customer’s sole and exclusive remedy is to discontinue its use of the TeamViewer software.
No Liability for Unauthorized Third-Party Access: The Customer further acknowledges that MXR Imaging shall not be held liable for any unauthorized access to the Customer's system or data by any other third party, or any unauthorized cyber security breach, where such access is gained through the TeamViewer application, unless such access is directly attributable to MXR Imaging's gross negligence or willful misconduct during an active and authorized remote support session.
Customer Responsibility: The Customer retains sole responsibility for maintaining the security of their systems, data, and network, including implementing appropriate security measures and monitoring access, while downloading the TeamViewer software.
